Registering with Us

We are proud of the facilities and services that we offer. We work very hard to make sure that every patient feels comfortable and relaxed in our care, and we welcome new patients into this environment.

Our practice list is open and we are accepting new patients that live in our practice area. We also are able to provide services for patients visiting the area temporarily. Please check our Temporary Residents page for more information.

Practice Area

The practice area comprises the whole of the Dulais Valley and the Upper part of Swansea Valley, including the Villages of Crynant, Seven Sisters, Onllwyn, Dyffryn Cellwen, Coelbren, Abercrave, Pen Y Cae and Callwen.

If you change your name and address or telephone number, please inform us as soon as possible (how to tell us about a change of name, contact numbers or address). If you move out of the practice area, you will be required to register with another practice.

How to register as a patient on a permanent basis:

  1. You can collect a registration pack for each new patient from the surgery. We will need you to fully complete and sign the forms before you can be registered. This will enable us to obtain your records from your existing surgery.
    You can also download and print the Dulais Valley Primary Care Centre Registration Pack and NHS Wales Registration Form and hand them in to reception. If you a registering a child under the age of 5, there is an additional form to complete which will be handed to the Health Visitor.
  2. You will need to provide two forms of identification from the list below:
    – Over 18s – one of the following: Photo ID (passport, current or provisional driving license, military identification)
    Plus one of the following (dated within the last three months): Utility Bill, Telephone Bill (excluding mobiles), Council Tax, HMRC letter, DWP letter, or other benefit document, bank statement
    – Under 18s – birth certificate, NI number, current or provisional driving license, adoption certificate, bank statement
  3. Do you care for someone who could not manage without you or are you someone that is cared for?
    If yes, please take a look at the resources on our carers information page.
  4. Please note, it is advised that you request one full month of any of your repeat medication from your existing surgery. This will allow time for your records to be transferred over to us. We also recommend you enclose your repeat medication order slip with your registration form. If you have medication concerns, please contact us.
