It is essential that we are notified of changes to your address, telephone or mobile number. Please inform reception of any changes or submit a change of name or address form to the practice as soon as your details change.
If you move out of our practice catchment area you should not assume the Practice can continue to look after you. You will need to find a doctor in your new area.
The practice is able to issue 4 weeks of your repeat medication to help you transition to a new practice. Please let us know if this is required.
Please note failure to inform us that you are moving out of our area will result in you being removed from our patient list, and may delay your future care.
